Submit a referral for Support Coordination or Psychosocial Recovery Coaching. Our team will review your referral and be in touch to discuss next steps.
Complex Connect is a registered NDIS provider offering Level 2 Coordination of Supports, Level 3 Specialist Support Coordination and Psychosocial Recovery Coaching. We work with participants across Melbourne and via telehealth across Australia.

Who Can Submit a Referral
- NDIS participants self-referring
- Family members or carers on behalf of a participant
- Support Coordinators or LACs
- Allied health professionals and treating teams
- NDIS planners or other NDIA staff
What to Expect After Submitting
Once we receive your referral, a member of our team will review the information provided and contact you within 2–3 business days to discuss the participant's needs and confirm whether we are able to accept the referral. Submission of a referral does not guarantee service commencement.
