Specialist Support Coordination

Specialist Support Coordination is designed for NDIS participants whose circumstances involve significant complexity, risks or barriers to implementing their supports. At Complex Connect, we have extensive experience working with people whose needs extend beyond disability services alone. This includes situations involving complex mental health needs, acquired brain injury, behaviours of concern, forensic or justice-system involvement, housing instability and difficult transitions between services. Sometimes the challenge isn't simply finding the right provider. It's working out how several services, professionals and systems need to work together, particularly when responsibilities are unclear or existing arrangements aren't working. We take a practical, collaborative approach to understanding these challenges, identifying options and coordinating the supports needed to move forward. Specialist Support Coordination is funded through an eligible participant's NDIS plan and provides a higher level of coordination where specialist expertise is required.

Who This Support Is For

Specialist Support Coordination is available to NDIS participants whose plans include funding for this higher level of support. It is particularly relevant when a participant faces significant barriers to accessing services, managing risks or maintaining effective support arrangements. Examples of circumstances where Specialist Support Coordination may be appropriate include: - A participant preparing to leave hospital or another institutional setting who requires complex accommodation and support arrangements. - Someone with significant psychosocial disability whose mental health, disability and other services need coordinated involvement. - A participant with behaviours of concern whose accommodation or support arrangements are at risk of breaking down. - Someone with acquired brain injury or cognitive impairment who requires assistance coordinating multiple services and supports. - A participant involved with the justice or forensic mental health systems who requires coordinated disability support. - A situation where several providers are involved, but responsibilities are unclear or existing arrangements are not meeting the participant's needs.
